The number of Singaporeans who are doing Advanced Care Planning (ACP) has risen. The Ministry of Health (MOH) told CNA that 4,500 instances of ACP were completed in 2018 which is more than double the number from 2015. ACP is a legally binding document involving a series of conversations to address one's preferred health and personal care options to ensure that prior planning is made should there be any unexpected medical crisis that would arise. Find out more from CNA online here.
Click here to learn more about ACP, including training details to become a Certified ACP Facilitator, or to request for ACP materials for your patients. You can also contact AIC at firstname.lastname@example.org or 6632 1199 to find out more.
