CareInMind - online platform that coordinates referrals, supporting GPs with Mental Health patients

​Last year, Dr Rodney Lim, a partner of Mental Health GP Partnership for over 10 years, helped to stabilise a patient, Mdm T’s, mental condition. Mdm T exhibited symptoms of hallucination and anxiety. Her symptoms were first noted by the Singapore Police Force who attended to her complaints on her neighbour’s spying activities. She was initially referred to a community allied health team for psychosocial assessment and subsequently to Dr Rodney for clinical treatment. With Dr Rodney’s dedicated care and expertise, and the support from the allied health team, Mdm T’s showed marked improvements and her condition stabilised within 2 months. 


While mental health conditions are commonly presented in primary care, over years of practice, Dr Rodney has realised that such issues are frequently neglected or unattended to. This has fed his interest in addressing this need for his patients. Aside from managing the treatment of those who are stabilized and right- sited from restructured hospitals, Dr Rodney also actively attends to walk-in patients who present with signs and symptoms of mental health conditions. His accumulated knowledge, experience and support received through the programme  fine-tuned his skills in identifying symptoms of mental health conditions and in crafting suitable care plans for these patients. 

Supporting Dr Rodney in making this positive change in Mdm T's life is CareInMind, a one access point for community partners. CareInMind is an online platform that coordinates referrals from the community and public agencies for clients who are suspected to have, or have mental health conditions. CareInMind engages GPs at the community level and renders their expertise for the benefit of patients who are then able to receive care for both physical and mental health needs close to home. 

The key services of CareInMind include:

 1. Triaging cases,

2. Linking clients to the appropriate community/mental health service(s),

3. Coordinating the patients’ care among the various stakeholders. 

The detailed coordination through referrals and updates that CareInMind provides exclusively for GPs who are part of the programme such as Dr Rodney, has been vouched as helpful. With CareInMind, Dr Rodney and the allied health team were able to receive timely updates about Mdm T’s progress. This enabled closer collaboration to better support Mdm T and her family. Ultimately, the joint effort of various parties made it possible for Mdm T to be successfully supported in the community. This service is in line with one of MOH’s focus areas, in Beyond Healthcare 2020, which looks into creating a seamless system that supports the care needs of a person holistically.
