The Functional Assessment Report (FAR) assess the need for assistance in Activities of Daily Living (ADLs) and is used to apply for a number of schemes administered by Agency for Integrated Care, SG Enable, Special Needs Trust Company and Housing Development Board.

They include the Pioneer Generation Disability Assistance Scheme (PioneerDAS), Foreign Domestic Worker (FDW) Grant, FDW Levy Concession for Persons with Disabilities (PWDs), Public Transport Concession for PWDs, Special Needs Savings Scheme (SNS), and Enhancement for Active Seniors (EASE).
It needs to only be filed once by the person needing assessment or caregiver (Section A of the FAR) and by doctors (Section B).
