ABOUT THE PROGRAMME

Supported by the Ministry of Health, the Singapore National Asthma Programme (SNAP) was established in 2001, to address the burden of asthma in Singapore.

 

In 2022, SNAP-3 aims to support asthma care in primary care, starting with the polyclinics, through the identification, tracking, and recalling of patients who should be on regular asthma follow-up, but default on appointments.


Mission: To develop sustainable, integrated asthma programs with community partners in Singapore

Vision: To provide innovative and high-value asthma care to all asthma patients
